We have provided some notes here to help you fill in our online application form.
Please read through them carefully to familiarise yourself with the application process and ensure you have all the information needed to hand.
These notes are repeated on the application form itself to guide you as you work on it.
You will have the opportunity to save your application and return to it later whilst you are completing it.
To apply online you must first log-in to the application system. If you have not used it before, select 'new applicant registration ' to create your account and log-in details. You will need to give your name, gender, date of birth, country of birth, email address and set up a 5 character password. If you have logged-in before and are returning to complete an application that is in progress, or are making a different application, just enter your existing log-in details to be recognised.
After you have entered these details once you will not need to enter these again as we will put them into your application for you automatically as you proceed. You will be asked to confirm your password and then given an option to upload a passport-sized photograph of yourself . If you are unable to do this, you can either bring in a photo with you on enrolment day or have us take one whilst you’re here.
Once you have completed these preliminary questions you need to select ‘New Application’ from the menu at the top of the page to proceed with making your application. You will not be asked for this information again when you next log-in, but if you need to update the information you have already given us you can return to it by selecting 'My Details’ from the menu at the top or by logging-in to the application site again.
You can now process with you application by selecting ‘Next’ from the menu at the bottom of the page.
Here you can select your programme of study from the drop down menu. If your course does not appear on this list then it is not a course you can apply for online. Return to the course page on the website where you can download an application.
The details you provide in the preliminary questions will be automatically entered into your application. If you need to change any of these details you can select ‘My Details’ at the top of the menu
- Home Address: Enter your full postal address in the box provide and your Postcode in the line provided below that.
- Term Address: If you have a different address during the academic year please enter this address as well as your home address. If you enter a term address we will use this as your mailing address instead of your home address. Enter your full postal address in the box provide and your Postcode in the line provided below that.
- Daytime Telephone number: Enter the phone number that we are most likely to be able to reach you on whether it is home or work. Remember to include any national or international codes.
- Mobile number: If you do not have a mobile phone number, enter another number either work or home which we can reach you on. Remember to include any national or international codes.
- Citizenship: You need to indicate whether you are UK, EU or Non-UK/EU citizen
- Nationality: You need to indicate your nationality by selecting form a drop down menu. You should choose the same nationality as is shown in your passport.
- Country of Permanent Residence: Select from the drop down menu the country you consider to be your permanent as in the country you are settled in.
- Please state country/countries of residence since 1st September 2006: Use the box to enter which countries you have resided in or lived in over the last 3 years dating back to September 2006. You can use the calendar in the box to indicate the dates you resided in 1 or more countries during this time. By selecting ‘add more’ you can enter as many lines in this box as are required.
If you are a non-EEA national, what is your immigration status? Non-EEA students are those students whose country of birth is outside the European Economic Area (EEA). Your country of birth or your nationality is what is written in your passport. If you are a non-EEA student we need to know what country you are from and then what type of visa you have to allow you to be in the country. If you do not have a visa that allows you to be in the UK and you intend to apply for a UK student visa, you need to indicate this here. We will not be able to provide you with any documentation to support your student visa application until after you have applied and been accepted on a course.
If you are a non-EEA national who requires sponsorship from the Trust under Tier 4 of the Points Based System, you will be required to provide English language test results at a minimum level B2 the Common European Framework of Reference for Languages (CEFR).
If you are a non-EEA national who does not require sponsorship from the Trust under Tier 4 of the Points Based System you will still be required to produce your passport and evidence of immigration status showing that you are entitled to study in the UK for the entire duration of your course. You will not be permitted to start on the course unless these documents are produced.

Word Limit – 300-700 words

For example:
You should give a brief description of your current role and responsibilities. You should say what specifically interests you about the programme you have chosen. Say why you want to train in this area and why you have chosen the Tavistock & Portman NHS Foundation Trust. How does it fit into your future plans for employment or further training? What knowledge of the subject do you have already? What have you read about it, do you have some practical experience of it, or have you trained in something similar before? Tell us about your interests, activities and achievements. Do you have work experience or voluntary activity to tell us about, that might be relevant, or that will give us a fuller picture of you as a person?
For those applicants who are returning to education after some time this is where you can help us to assess your experiences of education and any specific difficulties you may have had.
In addition, most applications will be followed up with an interview where your application will be discussed. Course administrators will contact you regarding a time for interview once your application has been received and processed.

- Please discuss the aspects of your learning at Year 1 that have interested you and the aspects you wish to develop.
You will need to provide the name, job title and email address of two people who will provide a written reference in support of your application. Applicants need to provide their referees with a reference request form. Forms can be downloaded from the website or by following the link. You can process with your application form once you have entered the details in this section. You can follow up on your written references after you submit your application form. Applicants are responsible for ensuring all written references are provided before they can begin their course.
As a condition of enrolment, all students enrolling on a clinical course or course involving observation of children or vulnerable adults will be required to apply for an up-to-date CRB Enhanced Level Disclosure. Disclosures made for other organisations will not be accepted.
A CRB disclosure should only be applied for after an applicant has been successful in being offered a place on a course and having accepted the offer.
